Do I need to fill out the health and immunization form?

Massachusetts law requires immunization and vaccination documentation to be on file in the Counseling and Health Service Office for students attending Salem State University, (as specified in 105 CMR 220.400). Learn more about the specific health requirements for students.

The following students are subject to the requirements of 105 CMR 220.600(C):

  1. Full-time undergraduate and graduate students
  2. Part-time undergraduate and graduate students in a health science program who may be in contact with patients
  3. Full or part-time student on a student or other visa, including a student attending or visiting classes or programs as part of a formal academic visitation or exchange program
  4. Please note that the COVID vaccine and booster requirement is for all students and employees on campus. This includes undergraduate and graduate students who are attending in-person classes, conducting research on campus, living in residence halls, participating in campus life activities such as student athletes or students with university sponsored visas.
