Salem State University must get your correct identifying number to file Form 1098-T, Tuition Statement, with the IRS and to furnish a statement to you. This will be your Social Security number (SSN) or, if you are not eligible to obtain an SSN, your individual taxpayer identification number (ITIN). Form 1098-T contains information about qualified tuition and related expenses to help determine whether you, or the person who can claim you as a dependent, may take either the tuition and fees deduction or claim an education credit to reduce Federal income tax. - Under federal law, you are required to provide the requested information. If you have been asked to fill out the W9S Activity Guide, please follow the following steps.
- Log into Navigator

Once logged in, click on the Task Tile.
If this is your first time logging in, it may take up to 24 hours before you will have access to complete this form. Complete all Tasks that are assigned to you. In this case, you will need to complete the W9S- Request Taxpayer SSN or ID by clicking on it.

Click on the W9-S Request Taxpayer SSN or ID that is coded as Assigned

Read all directions in Step 1, then select Next.

Verify all the information is correct on Step 2. Select the appropriate response and then complete all required fields:
- SSN here
- ITIN will need other form
- Acknowledgment at bottom
- Verify name and address

Once you fill in all the information, the below button will appear at the top. Click OK then Next.

Make sure you click the submit button on Step 3.
If you do not click submit, the Task will remain in Initiated status and will not be completed.
- Once complete, the Task will be removed from the list
