The School of Graduate Studies (SGS) is pleased to support our graduate students throughout their time at Salem State University. If you need guidance finding the correct form from the list below or considering your options in a particular situation, please contact us: graduate@salemstate.edu.  

You can find detailed descriptions of the forms and guidance about the documents you will need to upload as you complete the forms on the Graduate Student Information page in Canvas. 

Some of our forms require you to verify that you are an SSU graduate student. To do this, please follow these steps:

Funding your Education

We are excited when employers or other organizations support our graduate students. It shows an appreciation for you and an understanding of the value of graduate education. To facilitate third-party payments, we can provide a Billing Agreement form. Contact the Student Navigation Center (navcenter@salemstate.edu) to inform them about your situation and request a copy of the form.
